Idaho

Caregiving in Idaho.

Small rural population, no state PFL, no state estate or inheritance tax, and a relatively low cost of living that holds care prices below the national median in most settings. Idaho's caregiving landscape is shaped by long driving distances between facilities, a small but accessible Medicaid program, and the Treasure Valley (Boise/Meridian/Nampa) anchoring most senior services.

Three things to know right now.

01.Idaho has no estate, inheritance, or gift tax.

Idaho has never imposed a state-level estate tax and has no inheritance tax. The federal estate tax still applies above the federal exemption (approximately $13.99M per individual in 2025). For most Idaho families, estate planning focuses on probate avoidance, incapacity planning, and family coordination rather than state-level tax minimization.

02.Idaho Medicaid LTC: managed care for some populations, fee-for-service for others.

Idaho's Medicaid program uses a mixed delivery model. Some populations are enrolled in managed care plans (Blue Cross of Idaho, Molina Healthcare of Idaho, Idaho Medicaid Plus, depending on category); long-term services and supports may run through either managed care or fee-for-service depending on the specific waiver and population. The 5-year look-back is enforced. The Department of Health and Welfare administers Medicaid.

03.Federal FMLA is the only caregiver-leave protection in Idaho.

Idaho has no state paid family leave program and no state-level caregiver tax credit. Working caregivers rely on federal FMLA (12 unpaid weeks for employers with 50+ employees), accrued PTO, or employer-specific benefits. Idaho's small-employer workforce means many caregivers have no federal FMLA coverage at all and must rely on what their specific employer offers.

How we research Idaho-specific guidance.

Every state page is built from three sources: the state’s own statutes and regulatory filings, federal CMS and SSA documents that apply, and direct input from at least one credentialed reviewer who practices in or is licensed in that state. We re-review every state page quarterly. Idaho was last fully reviewed on May 21, 2026 by Reviewer to be assigned.
