Three things to know right now.

01.NC Medicaid is now managed care — Tailored Plans launched in 2024.

North Carolina transitioned most Medicaid beneficiaries to Standard Plans through Medicaid Managed Care in 2021, with Tailored Plans for behavioral health and I/DD populations rolled out in mid-2024. If your parent is dually-eligible or needs long-term services, understanding which plan applies matters more here than in most states.

02.No state estate tax, no inheritance tax, no state-level death tax of any kind.

North Carolina repealed its estate tax effective for deaths in 2013 and has never had an inheritance tax. Combined with a moderate income-tax rate and Social Security exemption, the state is increasingly competitive with Florida for retirees. Federal estate tax still applies for the few estates above the federal exemption.

03.Project CARE: a statewide free service for Alzheimer's caregivers.

North Carolina's Project CARE (Caregiver Alternatives to Running on Empty) is administered through the NC Department of Health and Human Services and partner Area Agencies on Aging. Counseling, respite vouchers (often $500-$1,500 per year), and family education at no cost to qualifying caregivers. Most eligible families never apply.
