Pennsylvania’s LIFE (Living Independence For the Elderly) program is the PA implementation of the federalPACE (Program of All-Inclusive Care for the Elderly). LIFE provides comprehensive medical, behavioral health, social services, and long-term services for participants 55+ who meet nursing-home level of care.3

How to evaluate a PA facility

  1. Confirm the license type (PCH or ALR) and what discharge conditions apply
  2. Check the most recent state inspection at dhs.pa.gov / online provider search
  3. For SNFs, cross-reference CMS Care Compare with PA Department of Health quality data
  4. Visit at least twice, including once unannounced
  5. Read the contract carefullyfor discharge terms — PCH discharge clauses are often less forgiving than families expect
