PCH vs ALR — the PA distinction
Pennsylvania’s 2007 Act 56 created the Assisted Living Residence license as a separate, higher-acuity category from the long-standing Personal Care Home license. Both are regulated by DHS:1
- Personal Care Home (PCH). 55 Pa. Code Ch. 2600. Provides shelter, meals, supervision, and assistance with ADLs. Cannot provide skilled nursing services. Residents must be ambulatory or capable of self-evacuation (with limited exceptions).
- Assisted Living Residence (ALR). 55 Pa. Code Ch. 2800. Higher-acuity. Can provide some skilled- nursing services, accommodate residents with greater impairment, and serve residents with progressive needs longer than PCHs can.
Practical implications:
- A PCH legally cannot retain a resident whose condition progresses beyond PCH capacity. Discharge is forced; the resident must move
- ALRs cost more per month but provide care continuity
- If your parent’s trajectory suggests ALR-level care within 2–3 years, select an ALR initially (or a facility holding both licenses)

For context: PA nursing-home pricing is mid-range nationally (national median ~$10,025) but well below the costs of NY ($17,200 metro) or CA ($15,178 statewide).
LIFE — Pennsylvania's PACE program
Pennsylvania’s LIFE (Living Independence For the Elderly) program is the PA implementation of the federalPACE? (Program of All-Inclusive Care for the Elderly). LIFE provides comprehensive medical, behavioral health, social services, and long-term services for participants 55+ who meet nursing-home level of care.3
Key LIFE features:
- Combines Medicare and Medicaid funding (for dual-eligibles) or private pay
- Coordinated care across all settings — physician, hospital, home care, day center, transportation, even prescription drugs
- Available in approximately 25 PA service areas
- Excellent fit for someone who would otherwise need nursing-home care but can remain in the community with comprehensive support
How to evaluate a PA facility
- Confirm the license type (PCH or ALR) and what discharge conditions apply
- Check the most recent state inspection at dhs.pa.gov / online provider search
- For SNFs, cross-reference CMS Care Compare with PA Department of Health quality data
- Visit at least twice, including once unannounced
- Read the contract carefullyfor discharge terms — PCH discharge clauses are often less forgiving than families expect
