Alabama

Caregiving in Alabama.

No state estate or inheritance tax, a strict Medicaid income-cap regime, and a homestead exemption that has not kept pace with home values — Alabama's caregiving landscape favors families who plan early.

  • Population 65+: approximately 900,000
  • Top metros: Birmingham, Huntsville, Montgomery, Mobile, Tuscaloosa

Three things to know right now.

01.Alabama Medicaid LTC is an income-cap program with a strict $2,000 asset limit.

Alabama uses the 300%-of-SSI income cap (~$2,901/month in 2026). If your parent's gross monthly income exceeds the cap, eligibility runs through a Qualified Income Trust (Miller Trust). The 5-year look-back is enforced without exception.

02.No state estate tax, no inheritance tax — federal only.

Alabama abolished its estate tax after the federal pickup credit ended in 2005 and has never had an inheritance tax. For most Alabama families estate planning is about probate avoidance and incapacity, not tax minimization.

03.Federal FMLA only — no state paid family leave.

Alabama has not enacted a state paid family leave program. Working caregivers rely on federal FMLA (12 unpaid weeks at employers of 50+) and whatever PTO their employer voluntarily provides. Plan for unpaid time off when intensive caregiving begins.

How we research Alabama-specific guidance.

Every state page is built from three sources: the state’s own statutes and regulatory filings, federal CMS and SSA documents that apply, and direct input from at least one credentialed reviewer who practices in or is licensed in that state. We re-review every state page quarterly. Alabama was last fully reviewed on May 21, 2026 by Reviewer to be assigned.
