Caregiving in California.

Largest senior population in the US, the most consumer-protective state for working caregivers, and the most consequential 2026 Medicaid story (the Medi-Cal asset test reinstatement). Plus Prop 19's inheritance ambush and the IHSS paycheck most CA families don't know about.

  • Population 65+: 6.0 million

Three things to know right now.

01.Medi-Cal's asset limit returned January 1, 2026.

From 1/1/2024 through 12/31/2025 there was no asset limit for Medi-Cal long-term-care eligibility. On January 1, 2026 the limit returned at $130,000/individual. Transfers made during the 24-month no-limit window are permanently excluded from the look-back. Families who planned during that window should confirm paperwork; families who didn't have a narrower toolkit now.

02.Prop 19 reshaped inherited property tax — most children lose the parent's base year.

Effective February 2021, an inherited home keeps the parent's low base-year property tax value only if the child moves in within 1 year as their primary residence, and only up to the parent's base + $1,044,586 (2025-2027). Inherited rental or vacation property is fully reassessed. The tax math has surprised more CA families than any other 2020s law change.

03.IHSS can pay an adult child for caregiving — many families don't realize.

California's In-Home Supportive Services program lets a Medi-Cal-eligible parent hire and pay an adult child (not a spouse or parent-of-minor) as their in-home care provider. At 2026 county rates of $16.90–$23.00/hour and up to ~283 hours/month for highest-need recipients, IHSS is one of the highest-leverage moves in the CA caregiver toolkit.

How we research California-specific guidance.

Every state page is built from three sources: the state’s own statutes and regulatory filings, federal CMS and SSA documents that apply, and direct input from at least one credentialed reviewer who practices in or is licensed in that state. We re-review every state page quarterly. California was last fully reviewed on May 21, 2026 by Reviewer to be assigned.
