Ohio

Caregiving in Ohio.

One of the largest aging populations in the Midwest — Ohio has a sophisticated Medicaid system, one of the largest dual-eligible managed care programs in the country (MyCare Ohio), no state estate tax (phased out in 2013), and a strong Ohio Department of Aging coordinating across 12 Area Agencies on Aging.

  • Population 65+: 2.2 million
  • Top metros: Columbus, Cleveland, Cincinnati, Toledo, Akron

Three things to know right now.

01.MyCare Ohio: one of the largest dual-eligible managed care demos in the US.

MyCare Ohio integrates Medicare and Medicaid benefits for dual-eligibles in 29 Ohio counties through five managed-care plans (Aetna Better Health, Buckeye Health Plan, CareSource, Molina, and UnitedHealthcare). The demo has been operating since 2014 and is one of the larger dual-eligible programs in the country. If your parent qualifies for both, MyCare Ohio is likely how they receive care.

02.Ohio phased out its estate tax in 2013 — and has no inheritance tax.

Ohio repealed its state estate tax effective for deaths on or after January 1, 2013, and has never had an inheritance tax. Only the federal estate tax applies for the small number of estates above the federal exemption. Combined with a moderate state income tax and Social Security exemption, Ohio is reasonably tax-friendly for retirees.

03.Ohio Department of Aging — well-funded statewide aging coordination.

ODA coordinates 12 Area Agencies on Aging covering all 88 Ohio counties, runs the PASSPORT waiver (Ohio's primary HCBS waiver), and operates the Ohio Senior Helpline. Combined with the long-running Ohio Long-Term Care Ombudsman program, Ohio has one of the more developed state-level aging-services infrastructures in the Midwest.

How we research Ohio-specific guidance.

Every state page is built from three sources: the state’s own statutes and regulatory filings, federal CMS and SSA documents that apply, and direct input from at least one credentialed reviewer who practices in or is licensed in that state. We re-review every state page quarterly. Ohio was last fully reviewed on May 21, 2026 by Reviewer to be assigned.
