01.Iowa inheritance tax: phased out through 2025.

Iowa was historically one of about six states with an inheritance tax. Legislation enacted in 2021 phased the tax out over five years — fully eliminated for deaths in 2025 and beyond. For 2026 deaths and forward, families should confirm with current Iowa Department of Revenue guidance, but the planning implication is that Iowa is now in line with the no-state-death-tax majority.

02.Iowa is rural — care logistics matter more than in metro-dense states.

More than 40% of Iowa seniors live in towns under 25,000. Adult day services, assisted living, and skilled nursing facilities can be 30+ miles from a parent's home. The Iowa Area Agency on Aging network and the LifeLong Links portal (211 partner) are the practical entry points; in-home care often beats facility care simply because facility care isn't local.

03.Iowa Medicaid HCBS Elderly Waiver — the path that keeps parents at home.

Iowa's HCBS Elderly Waiver (sometimes called the EW) provides Medicaid-funded home- and community-based services to seniors 65+ who would otherwise need nursing-home care. The waiver has historically had waitlists in some regions; check current status with the Iowa Department of Health and Human Services before relying on a specific timeline.
